Consent and Assent Forms (R2)

What this is
Four ready-to-use forms. Form A is parental or guardian consent for participants under 18. Form B is adult participant consent. Form C is photograph, video and media consent. Form D is medical and health information. All include the data protection wording.
When to use it
Use them once you know who is taking part. Form C is the one most projects forget until the photographs are already online.
